Phenobarbital in dogs: when to measure serum levels
5-year-old German Shepherd with idiopathic epilepsy that started oral phenobarbital every 12 hours 4 days ago. The owner asks when the first level should be measured.
Which monitoring schedule is appropriate?

A. 48 hours after starting, because its half-life is short
Phenobarbital has a long half-life in dogs (around 1.5–3 days). At 48 h steady state has not been reached and the value would underestimate the final level.
B. At 2 weeks (steady state), after every dose change and periodically, aiming for the therapeutic range
Correct. Steady state is reached at about 10–14 days. Repeat after every adjustment and periodically (around 6 months), together with biochemistry, aiming for roughly 15–35 mg/L.
C. Only if adverse effects or seizures appear
Individual variability is large: the same dose gives very different levels. Without measuring, you cannot tell whether the dog is underdosed or at risk of hepatotoxicity.
D. At 2 weeks, but it is of no value: levels do not correlate with seizure control
Levels guide adjustments: below the range, increase the dose before adding a second drug; above 35 mg/L the hepatic risk rises.
lightbulb Key takeaway
Phenobarbital induces its own metabolism, so levels can fall over the following weeks even without a dose change. Avoid gel separator tubes. Remember that it lowers total T4 without true hypothyroidism.
