Anal sac abscess in dogs: treatment
5-year-old Dachshund that has been scooting and licking its anal area for days. Today it has a fever and a hot, fluctuant, very painful swelling at the 4 o'clock position of the anus, with reddened skin that has not yet ruptured.
What is the most appropriate treatment?

A. Immediate anal sacculectomy to prevent recurrence
Operating on an infected, inflamed anal sac increases the risk of complications (fistulas, incontinence from sphincter damage). Sacculectomy is reserved for recurrence and done once the tissue is healthy.
B. Express the sac digitally every 2 days as for an impaction
With a closed abscess, squeezing is very painful and does not drain the cavity; manual expression is the treatment for a simple impaction.
C. Prednisolone to reduce the inflammation and antibiotic cream
Corticosteroids make no sense with a febrile abscess, and the cream does not reach the infected cavity.
D. Sedation, lance and drain the abscess, flush with dilute antiseptic, warm compresses, analgesia and systemic antibiotics because of the fever and cellulitis
Correct. The abscess is drained and flushed; fever and cellulitis justify systemic antibiotics. Flushing and warm compresses for a few days promote drainage and healing.
lightbulb Key takeaway
Impaction: manual expression. Sacculitis: expression, flushing and instillation of antibiotic/steroid into the sac. Abscess: drainage, flushing and systemic antibiotics. If recurrent, consider underlying atopic dermatitis or food allergy and, ultimately, sacculectomy.
