Neurogenic bladder in the paraplegic patient: how to manage it
Paraplegic patient due to a T3-L3 spinal cord lesion (dog or cat). The bladder is large and firm, urine dribbles by overflow and it is very hard to express manually.
What is the most appropriate management?

A. Bethanechol alone, to increase detrusor contraction so the patient urinates
In an upper motor neuron bladder the problem is increased urethral resistance. Raising detrusor pressure against a closed urethra does not empty it and can damage the bladder; bethanechol, if used, always goes with a urethral relaxant.
B. Continuous prophylactic antibiotics for as long as the paralysis lasts
Antibiotic prophylaxis does not prevent urinary infections in these patients and selects for resistant bacteria. Treat only clinically significant infections, based on culture.
C. Empty the bladder 3–4 times a day (expression, or sterile intermittent catheterisation if this fails) and use urethral relaxants such as prazosin or tamsulosin, and diazepam for the striated sphincter
Correct. Keeping the bladder small prevents detrusor atony from overdistension and reduces infections. Alpha-blockers relax urethral smooth muscle and make expression easier.
D. Do not intervene: overflow urination guarantees the bladder empties
Overflow dribbling indicates an overdistended bladder with a large residual volume, which promotes infection and causes permanent detrusor damage.
lightbulb Key takeaway
T3-L3 lesion: upper motor neuron bladder, large, firm and hard to express. Sacral lesion (L7-S3): lower motor neuron bladder, flaccid and easy to express. In cats expression is harder and catheterisation is more often needed. Teach owners the technique before discharge.
